As a courtesy to all of our patients, please call ahead to schedule appointments and arrive on time for your appointment so that we may keep waiting time to a minimum. New patients should arrive at least 15 minutes before their scheduled appointment to complete necessary medical history forms.
Confirmation and Cancellation
You will receive a phone call from our appointment reminder system, reminding you of your upcoming appointment. If you are unable to keep your scheduled appointment, please call our office 24 hours in advance. Your phone call is critical in helping us provide continuous care to all of our valued patients. If you should fail to give us notice of your cancellation, there may be a charge.Holidays:
